Open Enrollment School Year 2010-2011
Applications are now available for school year 2010-2011 at the Open Enrollment Office located in the Jr.High Building between the hours of 8:00-2:00
Applications can also be mailed upon request.
ALL applicants will need to provide the following documents along with the completed Open Enrollment application in order for the application to be processed.
THE APPLICANT MUST PROVIDE THEIR OWN COPIES OF THESE ORIGINAL DOCUMENTS – COPIES WILL NOT BE MADE FOR YOU.
· Birth Certificate of Applicant. Must be the Certified Health Department document. NO Hospital Birth Records will be accepted.
· Social Security Number.
· Report Card. Must be the MOST RECENT for the current grade level of the applicant. This does not apply for Kindergarten applicants.
· Shot Record.
· Custody Papers. If there has been a separation, divorce or if there is a Legal Guardian other than the parent(s) paperwork MUST be provided. If the applicants name listed on the Certified Birth Certificate does not match the Parent information – we will ask for documentation for proof of custody BEFORE the application will be processed.
· Proof of Residency. Current Utility Bill, copy of lease-rental agreement or mortgage statement. No substitutions will be accepted. Must be in the Parent/Guardians name.
· IEP and MFE. If the application is receiving ANY type of Special Education services, BOTH of these documents (copies) MUST be provided at the time of application.
Upon the receipt of ALL required documentation the open enrollment application will be
time and date recorded.
